A Roslyn man was charged on Tuesday in connection with an October 2014 incident in which he allegedly tried to fill a fraudulent prescription at a Hicksville pharmacy.

Narcotics Bureau detectives said Jason Kang, 30, of 78 Woods Drive, was arraigned in First District Court in Hempstead on two counts of criminal possession of a forged instrument.

Kang has been accused of presenting a falsified prescription for Oxycodone to employees of Smith’s Pharmacy in Hicksville on Oct. 8, 2014.

Police said the pharmacists identified Kang by his New York state driver’s license, which he submitted to fill the prescription, but he fled the scene before being confronted by employees.

On Tuesday, Kang surrendered to police around 10 a.m. and was placed under arrest, police said.
