Parker at Your Door clinical operations manager honored by United Hospital Fund

Parker Jewish Institute is pleased to announce that Parker at Your Door Clinical Operations Manager, Bonita Waites, a licensed practical nurse, was recently presented with a United Hospital Fund’s Tribute to Excellence Award.

The awards program, which this year was held virtually, recognized extraordinary personal leadership to improve quality of care, patient safety and patient experience. United Hospital Fund selected honorees by their hospital or long-term care organization for their vision and accomplishment.

As the clinical operations manager, Waites was honored for spearheading a chronic care management program serving frail, elderly and chronically ill patients. Waites proved instrumental in providing team-based, around-the-clock care management services, helping to prevent frequent emergency room visits and hospitalizations.

The United Hospital Fund recognition comes at a time when Parker at Your Door, which provides quality and convenient at-home medical care for older adults, is expanding to Brooklyn. The program already serves patients who live in Nassau, Suffolk and Queens counties.

In her role as clinical operations manager, Waites helped Parker at Your Door boost the health of older adults through a patient-centered care plan that is tailored to the patient’s specific needs, choices and values.

Under Waites’ leadership, this program improved overall outcomes for patients, including an increase in patient and family satisfaction, and a decrease in costly and stressful emergency room visits and hospitalizations.

“It’s these kinds of initiatives that allow New York’s growing older population to maintain their health and prevent a medical crisis,” said Michael N. Rosenblut, Parker’s president and CEO. “This program is helping community members age in place – safely and comfortably.”
