Northwell affiliate physician group recognized by American Heart Association

AdvantageCare Physicians (ACP), a multi-specialty physician group partner of Northwell Health, has been recognized by the local chapter of the American Heart Association (AHA) for participating in the Target Blood Pressure (BP) program.

This program distinguishes physician practices and health systems that achieve blood pressure control rates at or above 70 percent within the populations they serve. It is hoped that such efforts will lead to a reduction in the number of Americans who suffer heart attacks and strokes.

Northwell and ACP submitted blood pressure data gathered from 88 primary care physicians at 12 practices within the Nassau Queens Performing Provider System. This group is one of 25 New York State Performing Provider Systems that oversees the successful implementation of 11 Delivery System Reform Incentive Payment (DSRIP) program projects.

“Screening for high blood pressure and controlling it through a combination of lifestyle change and medication is one of the greatest investments one can make into his or her health,” said Mona Mittal, DO, senior director of health living disease management program Northwell Health.
